CSU Documentation Study: Survey description and disclaimers

Any information that is obtained in connection with this study and that can be identified with you will remain confidential and will be disclosed only with your permission or as required by law. No individual responses will be reported to anyone as all responses will be aggregated. The survey consists of 42 questions and should take approximately 20 minutes to complete. To ensure survey validity and consistency, I ask that only one survey be completed per department. The survey is designed to allow participants to exit at any time and reenter where they left off, up to the submission or the survey end date of Dec. 8, 2006.

Please be assured that participation is completely voluntary and that participants may stop at any point. Participants will indicate consent by completing the survey. The participant's decision whether or not to participate will not prejudice his or her future relations with Sonoma State University. If managers/coordinators decide to participate, they are free to withdraw their consent and to discontinue participation at any time without prejudice.

Once the survey period has ended, and with the consent of participants, I will conduct brief, follow-up telephone interviews with a random sample of respondants. All answers are considered confidential and neither the interviewee's identity, nor that of the library will be revealed.

I will ask several questions regarding participants' experience, education and training; the quality thereof will not, however, be questioned, nor will the quality of documentation they produce or manage. The survey results may suggest trends and common practices, but the survey primarily aims to recording the current state of documentation and knowledge management practices in the CSU libraries. I expect to make use of the survey results for a published article in a library journal such as Library Resources & Technical Services.
